How Does EMDR Work? What the Eye-Brain Connection Can (and Can't) Tell Us

If you've ever looked into EMDR therapy, you've probably wondered one thing: Why the eye movements?
It's a fair question. While EMDR has been extensively researched and is recognized as an effective treatment for trauma, scientists are still working to fully understand the mechanisms behind it. As research continues to evolve, one fascinating area of neuroscience has sparked curiosity: the relationship between our eyes and our brains.
You may have seen headlines claiming that the back of the eye contains brain cells. While this is often presented as a surprising new discovery, neuroscientists have actually known for decades that the retina—the light-sensitive tissue lining the back of the eye—is part of the central nervous system. During fetal development, the retina forms as an extension of the brain, making it unique among our sensory organs.
This doesn't explain EMDR on its own, but it does highlight something important: our visual system is deeply integrated with the brain.

How Does EMDR Work? The Science Behind Eye Movements
EMDR stands for Eye Movement Desensitization and Reprocessing. During treatment, you'll briefly bring a difficult memory to mind while engaging in bilateral stimulation, which often involves following your therapist's fingers with your eyes. Some therapists use alternating taps or tones instead, and research suggests these methods can be similarly effective.
The eye movements themselves aren't what "heal" trauma. Rather, they appear to support the brain's ability to process experiences that have remained emotionally overwhelming or unresolved.
Researchers have proposed several explanations for why bilateral stimulation may help:
It may reduce the vividness and emotional intensity of distressing memories by engaging working memory.
It may activate the brain's natural orienting response, allowing the nervous system to respond with curiosity rather than threat.
It may facilitate communication between brain networks involved in memory, emotion, and attention.
It may support the reconsolidation of traumatic memories so they can be stored in a less distressing way.
No single theory has been proven to fully explain EMDR, and it's possible that several of these processes are occurring simultaneously.
What Does the Retina Have to Do with It?
The retina's connection to the brain doesn't prove why EMDR works. That's an important distinction.
Instead, this research reminds us that vision is much more than simply seeing. Before visual information ever reaches the parts of the brain responsible for conscious perception, it has already been processed by specialized neurons within the retina. Our visual system is constantly interacting with broader brain networks involved in attention, emotion, memory, and regulation.
Scientists are also exploring whether changes in the retina may reflect changes occurring elsewhere in the brain. Researchers have investigated retinal imaging as a potential tool for studying conditions such as Alzheimer's disease, Parkinson's disease, multiple sclerosis, and other neurological disorders because retinal tissue offers a unique opportunity to observe living neurons non-invasively.
While this research is separate from EMDR, it reinforces a broader understanding that the brain functions as an interconnected system rather than a collection of isolated parts.
Healing Is More Than Talking About What Happened
One reason many people are drawn to EMDR is that it doesn't rely solely on talking through difficult experiences. Instead, it combines attention, memory, emotion, and sensory input in a structured way that helps the brain do what it was designed to do: integrate experiences that previously felt overwhelming.
For many people, traumatic memories remain "stuck." They continue to trigger the same emotional and physical reactions long after the danger has passed. EMDR helps the brain revisit those memories in a way that allows them to become less emotionally charged while preserving the facts of what happened.
The goal isn't to erase memories or convince yourself they weren't painful. It's to reduce the intensity of the distress they continue to create in the present.
Is EMDR Backed by Research?
Yes. EMDR is one of the most extensively researched treatments for post-traumatic stress disorder (PTSD) and is recommended by organizations including the American Psychological Association, the Department of Veterans Affairs, and the World Health Organization for the treatment of trauma.
Research also continues to explore its effectiveness for anxiety disorders, phobias, grief, panic, and other concerns, although the strongest evidence remains in the treatment of trauma-related conditions.
As neuroscience advances, we're learning more about how the brain stores memories, regulates emotion, and responds to therapy. While we don't yet have every answer about why EMDR works, the evidence consistently shows that it can help many people process experiences that have remained stuck for years.
